Do I need to obtain a claim form prior to receiving eye care services?

No. Taking advantage of your EyeMed discounts is easy and convenient. When visiting an EyeMed provider, there are no additional forms to fill out. Simply present your EyeMed Member ID card (if you've received one) at the time of your appointment.

If your plan includes an out-of-network option, you can request an out-of-network claim form by contacting Member Services at 1-877-226-1115. A representative will verify that your program includes this option before sending the out-of-network claim form to you.


Do I need a referral?

You do not need a referral before using your EyeMed benefits.


Is my family included?

You can include your entire household under most EyeMed plans, but we recommend that you refer to your plan information, or call your Program Administrator, to be certain.


I have an eye care professional that I already visit on a regular basis. How can I find out if he or she is an EyeMed provider?

To find out whether or not your current eye care professional is a EyeMed provider, do one of the following:

  1. Contact your eye care professional.
  2. Use EyeMed's online Provider Locator.
  3. Call 1-877-226-1115 and use our automated phone system to inquire about EyeMed providers.
  4. Refer to your Member brochure, which may list your nearest provider locations.

I have a lens prescription. Is an examination required before I receive eyeglasses or contact lenses?

To receive eyeglasses or contact lenses, you must have an unexpired prescription. (Like most medical prescriptions, prescriptions for eyewear expire.) Eye exams are an important part of your overall health as well as your eye health, so EyeMed recommends scheduled visits to an eye care professional.

If your prescription was recently written, we still recommend asking an EyeMed provider whether or not the prescription is recent enough to be filled. For contact lenses, the EyeMed provider may need to conduct some additional services for fitting and follow-up to ensure your eye health, comfort, and a proper fit.


How old should my child be before I schedule an appointment for an eye examination?

Although most people aren't aware of it, children should have their first eye examination at the age of six months. Even at six months, optometrists and ophthalmologists can evaluate a child's eye health and detect any visual conditions for early treatment. Please check with your Program Administrator or Membership information to make sure that your child's eye examination is discounted.


Can I receive an eye exam from one provider and materials (frames, lenses, contacts) from another?

Yes. Please refer to your plan coverage, or discuss your options with your Benefits Administrator before receiving services.

Please note: Specific services must be administered by your EyeMed provider prior to receiving contact lenses. Should you choose to visit a second EyeMed provider for contact lens services, some procedures may be duplicated to ensure a proper fit. Discuss procedures and fees with your initial EyeMed provider prior to seeking services at another participating EyeMed location.


My current eye care professional is not an EyeMed provider. Can he or she be contacted by EyeMed to request participation in the EyeMed plan?

EyeMed is happy to contact your current eye care professional and extend an application for participation. Email us your eye care professional's name, address, and phone number, along with your name and phone number to use as a reference. In the meantime, visit our online Provider Locator to determine which EyeMed network providers are most convenient for you.


I have a question about the services or treatment I've received. Who should I contact?

Member satisfaction is our top priority. If you have any questions or concerns, please contact your EyeMed provider, or contact us. You may also call a Representative toll-free at 1-877-226-1115. Please provide your name, phone number, date of service, and all details when registering a question or concern.
